**Case Study: Intensive Monitoring of Elevated Cardiac Stress Markers and Metabolic Abnormalities During Neonatal Jaundice Treatment**

Newborn jaundice is quite common shortly after birth. However, in some cases, other systemic abnormalities can emerge during treatment, requiring careful observation.

This case involves a 4-day-old infant referred to our clinic for phototherapy to treat jaundice. During the course of treatment, our medical team detected elevated cardiac stress markers, progressing anemia, hypoalbuminemia (low protein levels), and metabolic abnormalities. The newborn was admitted for intensive monitoring and a comprehensive evaluation to ensure their complete safety.

Following a recommendation for phototherapy, the family visited our outpatient clinic with a medical referral. Upon admission, the baby’s total bilirubin was measured at 16.3 mg/dL.

  Diagnosis & Step-by-Step Evaluation

1) Jaundice Treatment & Monitoring Immediately upon admission, we initiated phototherapy. The newborn responded well to the treatment, and jaundice-related levels began to steadily improve. We closely monitored the baby’s feeding tolerance and bowel movements alongside the therapy.

2) Detection of Systemic Abnormalities & Further Evaluation Beyond the jaundice, our comprehensive inpatient testing and continuous observation revealed several underlying concerns:

Because of these findings, our medical team determined that simply discharging the baby after completing phototherapy was not safe. We opted to continue hospitalization for expert monitoring, further diagnostic testing, and stabilization to identify the root causes of these symptoms.

3) Infection Screening & Continuous Monitoring While there were no distinct signs of a severe infection, a newborn’s immune system is incredibly fragile, making it difficult to completely rule out latent infections. To ensure absolute safety, we conducted targeted additional screenings within necessary limits and continued to track the infant’s progress.

Although the jaundice showed a clear trend of improvement, the presence of systemic issues meant that extended inpatient observation and thorough diagnostic checks were crucial to confirming the baby’s overall stability.

While the infant was originally admitted for neonatal jaundice and responded beautifully to phototherapy, our vigilant monitoring caught hidden systemic abnormalities. Rather than simply concluding treatment once the jaundice cleared, our dedicated team prioritized identifying the underlying causes and closely monitoring the baby—ensuring the newborn was truly stable and safe before considering discharge.
